What the Latest Research Shows

As chiropractors, one tool we often use is thoracic spine manipulation—a targeted adjustment to the middle back that may influence areas like the neck, shoulders, and upper arms. But does it really work?
What Researchers StudiedA recent systematic review looked at 26 studies investigating thoracic high-velocity, low-amplitude thrust (Tx-HVLAT) manipulations. This technique involves a quick, controlled thrust to the thoracic spine, aimed at improving movement and reducing pain. Researchers focused on outcomes like:
Pain intensity
Disability or functional limitations
Patient-reported overall improvement
They also examined secondary outcomes such as range of motion, muscle strength, posture, and adverse effects.
What the Evidence Shows
Neck Pain:
Thoracic spine manipulation may provide short-term pain relief and improved function compared to sham treatments, general exercise, or passive therapies.
It seems less effective than a direct neck (cervical) manipulation in some studies.
Evidence quality is low to very low, meaning more high-quality research is needed.
Shoulder Pain:
Surprisingly, thoracic manipulation was not consistently better than sham treatments for shoulder pain or function in the short term.
Compared to general conservative care, there was very limited evidence of short-term benefits.
Cervical Radiculopathy (pinched nerves in the neck):
Some studies show thoracic manipulation may help with pain and function, but the evidence is still very limited.
Cervicogenic Headaches:
There is low-quality evidence suggesting thoracic manipulation may reduce headache pain and improve neck function compared to doing nothing.
Temporomandibular Disorders (Jaw Pain):
Some immediate benefits for neck and jaw pain were noted, but again, evidence is limited.
What This Means for PatientsWhile thoracic adjustments may help with neck pain, headaches, and jaw pain, the research is still in its early stages. For shoulder pain, results are mixed, and some studies suggest the effect may be minimal.
