Tension-type headache (TTH) is the most common headache in the world — yet it’s often misunderstood.
If your headaches feel like a tight band around the head, a dull pressure, or constant heaviness that isn’t worsened by exercise, you’re likely experiencing tension-type headaches.
📈 Research shows that cases of TTH in people aged 15–39 increased by 37% between 1990 and 2019, and they’re more common in women.

Episodic vs Chronic Tension Headaches
Tension headaches aren’t all the same.
🔹 Episodic TTH: fewer than 15 headache days per month
🔹 Chronic TTH: 15 or more headache days per month
As headaches become more frequent, they often change in nature:
Muscles become more tender
Pain thresholds drop
The nervous system becomes more sensitive
Daily function and quality of life decline
This shift isn’t just about more headaches — it reflects changes in how the brain, nerves, and musculoskeletal system interact.
How the Neck and Spine Can Trigger Head Pain
One of the most important discoveries in headache research is the role of the trigeminocervical system — a shared nerve pathway where sensory information from the upper neck and head converge.
🧠 This means:
Pain from the neck, muscles, joints, or fascia can be perceived as head pain
Ongoing input from irritated tissues can sensitise the nervous system
Headaches may continue even when no “damage” is present
In short:👉 A sensitive neck can drive head pain👉 A sensitive nervous system can amplify it
What This Study Looked At
This recent study compared women with tension-type headaches to women without headaches and examined:
✔ Neck posture✔ Neck and spinal mobility✔ Neck, trunk, and core muscle endurance✔ Whole-spine alignment (from neck to pelvis)
Importantly, participants had no major medical conditions, making the findings highly relevant to everyday headache sufferers.
Key Findings (In Plain English)
1. Neck posture wasn’t the problem — movement and endurance were
Despite common assumptions, women with tension headaches did not have worse forward head posture than those without headaches.
However, they did have:❌ Reduced neck flexion and extension mobility❌ Poor endurance of the neck flexor muscles (especially deep stabilisers)
This suggests that how the neck moves and stabilises matters more than how it looks.
2. Deep neck muscles were significantly weaker
Women with TTH showed large reductions in:
Neck flexor endurance
Deep neck stabiliser performance
🧠 These muscles play a key role in:
Head and neck control
Proprioception (body awareness)
Reducing overload on superficial muscles like the upper traps and SCM
When they fatigue easily, the neck becomes less efficient — increasing strain, muscle tenderness, and headache sensitivity.
3. The problem doesn’t stop at the neck
The study also found that women with TTH had:
Increased thoracic kyphosis (rounded upper back)
Reduced endurance of trunk flexors
Reduced side core (lateral flexor) endurance
Poorer core stability scores
Interestingly:✔ Lumbar posture and mobility were normal✔ Spinal mobility outside the neck was largely unchanged
This suggests the body adapts locally, particularly around the neck–upper back junction, rather than the entire spine breaking down.
Why This Matters Clinically
According to the motor adaptation to pain theory, the body adopts protective movement strategies in response to pain — even in areas away from the painful site.
In tension headaches, this may look like:
Reduced neck movement to “protect” the head
Increased reliance on superficial muscles
Subtle changes in upper-back posture
Reduced core endurance over time
These adaptations are not damage, but they can:🔁 Maintain headache sensitivity🔁 Increase recurrence🔁 Reduce resilience under stress
Chronic Headaches Don’t Mean Structural Damage
A key takeaway from this research is reassuring:
👉 Tension headaches are not caused by one damaged structure👉 Muscle dysfunction is a contributing factor, not the root cause👉 Pain is driven by the interaction between muscles, movement, and the nervous system
This is why scans often show nothing alarming — and why movement-based care is so effective.
What Helps Most (According to Research)
The evidence supports:✔ Improving neck mobility✔ Training deep neck flexor endurance✔ Addressing upper-back posture and control✔ Building trunk and core endurance✔ Reducing fear around movement
