웰니스 멤버십 이용약관
1. 예약 가능 시간
월요일~금요일 오후 9시 30분~5시 30분
2. 가족 약속은 함께 이루어져야 합니다(청구 목적의 가족은 최대 성인 2명과 어린이 2명입니다).
3. 선불만 - 진료 월말이 아닌 진료 월초에 납부해야 합니다.
4. 주간진료는 그 달에는 일주일에 한 번 오시면 된다는 뜻입니다. 격주 진료는 14일에 한 번씩 오실 수 있다는 의미입니다.
5. 해당 월의 진료는 사전 예약이 필요합니다.
6. 취소 통지가 24시간 이내에 발생하는 경우 해당 주간의 치료는 무효화됩니다.
7. 공휴일 이용 불가
8. 해당 월 내에 추가 진료가 필요한 경우, 저희 팀은 웰니스 멤버십 환자에게 할인율을 제공합니다.
9. 멤버십 시작일은 매월 언제든지 설정할 수 있으며, 반복 만료일은 동일한 날짜가 됩니다.
